When you clear the outdoor crypt, you have 2 ways you can follow, either you help the friendly ghost and kill Guido and his wife, or kill the ghost. Detailed Walkthrough

The noble Guido of Porto Draco and his wife Katjana ask you to clear their mansion of spirits, with Guido promising you great rewards mentioning that he is a relative of DeMordrey. You clear the mansion and enter their outdoor crypt and slay some more ghosts, when at the end you meet Ghost of the Lady Janina, in her time the rightful ruler of Porto Draco, who was murderd by her sister Katjana and Lord Guido. She tells you that killing her will do nothing and the only way for her to rest is for her murderers, Lord Guido and Katjana, to die. You can then do one of two paths, either you help Lady Janina or you kill her.

If you decide to help her, you must go back to Porto Draco and confront Lord Guido and Katjana. Guido state that the ghostly whore Janina has clouded your mind and that he will ram two feet of the best Mascarell steel into your guts. After you (naturally) kill them instead, you head back to Ghost of the Lady Janina, she thanks you for avenging her, while pondering on their motives to have killed her. Lady Janina then tells you that her time here is running out and warns you that the danger from the south is much greater than that from the north. She then bids you farewell and disappears to start her well deserved eternal rest.

If you decide to kill her, [please fill in, haven't done this path]
